The Imperial Transition Program will empower our High/Low Incidents students achieve the greatest degree of independence and quality of life, through addressing the students’ individual skill deficits and enhancing their overall understanding of the “post-school world” critical skills and concepts. Our goal is to prepare the student for a successful integration into the community they desire.

 

Our aim is to prepare the student for a successful integration into the community as an effective adult by infusing a person-centered approach throughout three components:

  • Work Skills: career exploration; worker traits; job development; job training, co-worker relationship; and all areas related to employment

  • Personal Management: self-advocacy; daily planning; budgeting of time and money; shopping; cooking; home and personal care.

  • Community Access: transportation; joining and expanding community programs and/or recreation; creating peer relationships and other lifelong activity.

 

It has also been our honor to work with court-involved youth. The Imperial Transition Program participates in creating a successful transition accountability plan. We improve outcomes for students with disabilities transitioning from an institution (Juvenile Justice System) to their community and we would be honored to be a part of the transition team by providing them with one of our sound transition plans. Each plan is rooted in the transition practices reviewed by Wagner and Davis (2006):

 

 

  • Support young people in creating meaningful relationships that engage them in their education.

  • Provide a sense of rigor and challenge for youth so that, with adequate support, they will complete a program and gain a sense of accomplishment.

  • Require youth to have opportunities for real-life learning of skills relevant to their transition.

  • Focus on the whole child successful transition because any neglected area can have negative spillover effects into others.

  • Ensure that transition planning be person-centered, involving the youth and other people important to them in the process.
